The second Dahlgren (DD-187) was launched 20 November 1918 by Newport News Shipbuilding Co., Newport News, VA.; sponsored by Mrs. J. Pierce, daughter of Rear Admiral Dahlgren; and commissioned 6 January 1920, Commander L. Sahm in command.

Dahlgren joined the Atlantic Fleet for exercises and training along the east coast, in Mexican waters, off Guantanamo Bay and in the Canal Zone. She took part in the Presidential Fleet Review at Norfolk in April 1921, and in bombing tests on former German ships off the Virginia coast that summer. On 30 June 1922 she was placed out of commission at Philadelphia.

Recommissioned 26 October 1932, Dahlgren stood out of Norfolk 7 November for San Diego, arriving 30 November. Destroyer operations engaged her along the west coast until April 1934 when fleet exercises brought her to the Atlantic. In January 1936 she returned to San Diego. After a period of similar operations on the west coast, she sailed again for the east on 1 July 1937 and having rescued the crew of a Coast Guard Coast Guard seaplane arrived at New York 21 July 1937.

She served in engineering experiments until 14 June Dahlgren sailed out of Norfolk and Newport on patrols and escorted submarines in their training, and from January to 1 April 1941 she served in the Patuxent River, Md., in experiments in ordnance and submarine detection. Through the summer of 1941 she tested a variable pitch propeller, and subsequently escorted a new cruiser during her trials. On 4 January 1942 Dahlgren arrived at Key West to escort Washington (BB-56) in operations in the Gulf of Mexico. She returned to New York 8 February for a brief period of coastal patrol, and on 24 March sailed to Key West to serve the Fleet Sonar School and carry out patrols. During these operations, she rescued 67 survivors of the torpedoed tanker SS Pennsylvania Sun on 16 July 1942, and nine survivors of the blimp K-74 on 19 July 1943.

On 11 January 1945, Dahlgren arrived at Charleston S.C. to operate with submarines in training until 1 March, when she was reclassified AG-91. She served the Mine Warfare Test Station at Solomons Island Md. until 16 November 1945 when she moored at Philadelphia Navy Yard. There she was decommissioned 14 December 1945, and sold 17 June 1946.

Dwight family papers

The Dwight Family Papers consist of correspondence, financial records, writings, and other memorabilia of Yale President Timothy Dwight (1752-1817), his ancestors, descendants, and other relations. Relatives in the Edwards, Hooker, Strong, Woodbridge, and Woolsey families are represented. The largest quantity of correspondence documents the family life of John Williams Dwight (President Dwight's grandson), his wife Sophia Dwight Dwight, and children Mary Clarissa, Edward Strong, and Emily Cecilia Dwight. The papers also include courtship letters of Mary Alsop Dwight Patrick and professional papers of President Dwight and his grandson, President Timothy Dwight (1828-1916). Though the papers date from 1713 to 1937, most of the papers were produced in the nineteenth century.

The library received the Dwight Family Papers from numerous donors between the 1940s and 1985, and additional materials have been purchased from other sources. Additional Dwight family material is housed in the Beinecke Rare Book and Manuscript Library. The two and one-half linear feet of papers are arranged in two series: I. Correspondence and Papers, 1713-1937 and II. Family Papers, 1788-1929.

Series I includes letters, financial and legal papers, and memorabilia arranged in chronological order, while Series II consists of other types of material arranged by individual. An index to selected correspondents is included at the end of this register.

Series I begins with legal and financial documents of the Dwight family in eighteenth century Northampton, Massachusetts, including deeds, accounts, and probate records (folder 1). There are also a 1748 account of W. B. Dwight's death and letters for Timothy Dwight (1694-1771) to the Rev. Dr. Thomas Foxcroft describing religious activities in the Northampton area. A 1795 letter invites Timothy Dwight to assume the presidency of Yale College, and several letters in folders 1-3 concern Yale matters. Other letters in these folders focus on family affairs and include exchanges between parents and children and husband and wife.

Folder 3 includes medical advice to Timothy Dwight (1816 Dec. 7) on treating a stricture of the urethra and a copy of the original minutes recorded by Benjamin Silliman at President Dwight's bedside on January 10, 1817, the day before his death. These minutes concern directions for the publication of Dwight's writings. Folder 3 also contains remarks by Dwight on the "comparative value" of his sermons and correspondence of his son Timothy Dwight (1778-1844) concerning the posthumous publication of his father's "System of Divinity". The folder also includes correspondence of Benjamin Dwight, Sereno Dwight, Mary Woolsey Dwight, and Theodore Dwight.

The majority of the correspondence in folders 4 and 5 relates to the courtship of Mary Alsop Dwight by Matthew A. Patrick, a career army officer serving in 1832 in South Carolina. His letters include comments on the nullification crisis, which delayed his wedding furlough. A few letters from Mary Alsop (Dwight) Patrick to her father Theodore Dwight describe her life as an officer's wife. Folder 5 also contains correspondence of Theodore Dwight, Jr., concerning the publication of his textbooks, and of Timothy Dwight's (1752-1817) heirs, concerning their claim for a Revolutionary War veteran's pension.

Correspondence between John W. Dwight and his future wife Sophia Dwight of Clinton, New York, begins in folder 5. There are numerous letters to "Sophy" in folders 6 and 7, and folders 8-12 are almost exclusively concerned with this branch of the Dwight family. For a period of a year and a half (1862-1863) John Dwight lived in New Haven, working at a saltpeter business, while his wife and family lived in Clinton, near her family. The almost daily letters between husband and wife concern family finances, weather, the eating habits and behavior of the children (Mary, "Minnie" or "Molly" Edward, "Ned" and Emily, "Cecily"), the death of son Louis, John's draft status, and the deteriorating nature of Sophia's health. Following Sophia's death in 1863 there are far fewer letters, but the correspondence through folder 25 is predominantly that of Mary and Edward with their father, now the president of the New Haven Fertilizer Company, uncle Benjamin W. Dwight, and other relations and school friends. Edward graduated from the Yale Medical School in 1886, and several letters concern attempts to establish his medical practice. From folder 17 on there are occasional letters from Timothy Dwight (1828-1916) to non-family members. The majority of these letters are dated after 1888 and concern Yale University business.

Folders 24 and 25 contain additional undated letters of John W. Dwight's family, while folders 26-29 include memorabilia, writings, and financial and legal papers of this same branch of the Dwight family. Folder 28 includes photographs of the Dwight family ancestoral property in Dudley, Massachusetts.

Series II includes papers of Margaret Dwight Bell, Benjamin W. Dwight, Henry Dwight, Timothy Dwight (1752-1817), Susan Edwards Daggett Dwight, and Timothy Dwight (1828-1916). The largest quantity of material relates to Timothy Dwight (1752-1817) and includes sermons and other writings, as well as notes of his sermons and discourses recorded by his son Benjamin W. Dwight, Alexander Phoenix, and an unidentified student in the Yale College seniors' recitation room. Folders 43 and 44 contain Dwight's journals of travels to Cape Cod, Newport, Portsmouth, and Vermont. These were later edited and form a small portion of the published version of Dwight's Travels in New England and New York. (A few additional journals are found in the Humphreys-Marvin-Olmsted Papers, MS 857.) The series includes both printed and manuscript sermons, many of which were delivered at Yale College, and Dwight's poetry.

Benjamin Dwight is represented through biographical writings on his father, some travel observations, and his account of a storm in the Catskills, which he copied and sent to Benjamin Silliman in 1820. Margaret Dwight Bell's papers relate to her 1810 journey from New England to Warren, Ohio. The handwritten copy of her original journal describes the bad roads, inns, and travelling companions she encountered. For Henry Dwight there is a notebook, in German, from studies abroad and a book of hand colored flower engravings. Susan Dwight's papers consist of an autograph album, and for Timothy Dwight (1828-1916) there is a copy of his address at the dedication of Osborn Hall and inscriptions from books presented by him to Battell Chapel. The series concludes with a box containing six pairs of eyeglasses which belonged to Dwight Family relatives.

Minutes count

Speed of treatment is a critical factor in determining the outcome of thrombolytic and/or endovascular treatment for patients with disabling acute ischemic stroke. In this regard, it may be useful to consider how the treatment of myocardial infarction (MI) has evolved (see Box 4). 4 , 6 , 32 – 36 Rapid diagnosis of acute MI is facilitated principally by ECG. Measurement of serum enzymes (creatine kinase MB fraction and troponin T/I) is not useful in the first hour. The clinical history, physical examination and ECG findings are the diagnostic mainstays. In some large cities, paramedic personnel perform ECG and can transmit the tracing electronically to the attending cardiologist, which allows treatment decisions to be made en route. In some jurisdictions, ambulance-based thrombolysis is initiated on this basis alone. 37 , 38 Electronic transmission of the ECG tracing has also facilitated a direct-to-catheterization-laboratory approach for primary percutaneous coronary intervention, with bypass of the emergency department and reduction in door-to-balloon time, which has led to better outcomes for patients with MI. It remains true that when delays to primary percutaneous coronary intervention are expected (e.g., because of transport distance), it is better to treat rapidly with intravenous thrombolysis. 37

Box 4:

Acute revascularization for ST-segment elevation myocardial infarction 32

Patients with ST-segment elevation myocardial infarction (STEMI) most likely have a large, acute occlusion of the coronary artery that will be amenable to emergent revascularization with thrombolysis or primary percutaneous coronary intervention (PCI). These patients represent about 40% of all cases of acute coronary syndrome. 4 , 33 Initial thrombolysis trials dating back to the 1970s had neutral or negative results. In 1986, with the publication of the landmark GISSI Trial (Gruppo Italiano per la Sperimentazione della Streptochinasi nell’Infarto Miocardico), thrombolysis entered the mainstream. 34 The GISSI trial showed that the sooner streptokinase was administered, the better were the chances of recovery, with significant improvement in mortality rates. Numerous other clinical trials followed, reinforcing the evidence for rapid administration of streptokinase. Eventually, streptokinase was supplanted by newer agents, such as recombinant tissue plasminogen activator (alteplase) and tenecteplase, which have superior efficacy, with a treatment window of six hours from symptom onset. These trials also resulted in the concept of door-to-needle time as a measure of system performance.

The limitations of systemic thrombolysis for STEMI, including significant decrease in efficacy with prolonged ischemic time, failure of reperfusion, risk of reinfarction and an approximately 1% risk of intracranial hemorrhage, prompted development of local therapies: first endovascular thrombolysis and then PCI. 35 Primary PCI is now the preferred strategy, but only where coordinated systems are available to ensure timely access. For patients with STEMI, immediate transfer to a PCI-capable institution is recommended, provided this transfer can be achieved within 90 minutes and provided that total ischemic time is less than 12 hours. If this is not feasible, and particularly for patients who present within two to three hours of symptom onset, thrombolytic therapy should be given before arrival at the hospital or at the non–PCI-capable hospital, followed by transfer to a PCI-capable centre.

Coronary care units are the cornerstone of post–myocardial infarction (MI) care. The development of this type of unit began in the 1960s, the first in Toronto being established in 1965. By the 1980s, when the evidence for thrombolysis for acute MI became accepted, infrastructure was already in place for treatment of acute MI, which made the goal of short door-to-needle times highly feasible. Furthermore, admission to a coronary care unit is beneficial because this setting offers rapid identification and treatment of malignant arrhythmias, diagnostic expertise and early introduction of medical treatments. 6 , 36

In cases of suspected ischemic stroke, bypassing the emergency department is difficult because of the need for imaging. The particular stroke type (ischemic v. hemorrhagic) may be suspected before arrival at the hospital, but cannot be confirmed without imaging. Rapid in-hospital processes and a team approach are required to reduce door-to-needle times. A median time of 20 minutes was shown to be possible in a cohort study from Helsinki, 39 but in North America median times of 60 minutes or more are typical. 30 On average, for every minute of delay after middle cerebral artery occlusion, 1.9 million neurons, 14 billion synapses and 12 km of myelinated fibres are destroyed. 40 Efforts to reduce door-to-treatment times for stroke are continuing. Research studies are underway in Berlin and Houston to examine the effects of placing CT scanners in ambulances to allow rapid imaging, remote transmission of imaging data for expert review and a thrombolysis decision, so that paramedics can deliver thrombolytic therapy in the ambulance. 41 Ambulance-based CT may eventually facilitate a direct-to-angiography-suite approach for acute ischemic stroke, similar to what is already in place for MI.

Hours count

Distinguishing clinically between transient ischemic attack and minor stroke is not particularly helpful for prognosis because both are associated with substantial early and 90-day risks of major ischemic stroke, and urgent investigation and management are therefore warranted for both. 15 , 42 , 43 Cohort studies have shown that up to one-third of inpatients with a clinical diagnosis of transient ischemic attack (no residual deficits on examination) have evidence of ischemia on MRI. 11 , 44 Almost all patients with minor stroke have small infarcts that can be visualized by MRI. 45 In this way, transient ischemic attack and minor stroke are conceptually similar to non-ST-segment elevation MI (Box 5). 46

Box 5:

Non-ST-segment elevation acute coronary syndromes 46

Non-ST-segment elevation acute coronary syndromes include unstable angina and non-ST-segment elevation myocardial infarction. Unstable angina is considered an acute coronary syndrome in which there is no detectable release of enzymes (biomarkers of myocardial necrosis). Most patients with acute coronary syndrome have recurrent transient reduction in coronary blood supply caused by formation of thrombus at the site of atherosclerotic plaque rupture and/or vasoconstriction. These events occur as a consequence of platelet aggregation and complex interactions among the vascular wall, leukocytes, platelets and atherogenic lipoproteins. Non-ST-segment elevation myocardial infarction has the same pathophysiology as unstable angina but is distinguished by the release of cardiac markers of myocardial necrosis (e.g., troponin T). Management involves early risk stratification and rapid institution of medical treatments: antiplatelet agents, antithrombotic agents, β-blockers and statin therapy. Thrombolytic therapy is not indicated for these patients. Early coronary angiography and revascularization are beneficial in high-risk patients with acute coronary syndrome.

The risk of disabling stroke after transient ischemic attack or minor stroke is substantial and is heavily 𠇏ront-end loaded.” 47 Recognition and early (i.e., same-day) evaluation and treatment may substantively reduce this risk. 48 – 50 Dual antiplatelet regimens for the short-term are warranted and continue to be investigated. 51 , 52 Carotid artery revascularization and anticoagulation for atrial fibrillation are proven effective treatments that prevent stroke. 53 , 54 For patients without these conditions, transient ischemic attack presents a golden opportunity to prevent future stroke by intervening on lifestyle and vascular risk factors. 31

Symptoms and Physical Findings

The diagnosis of an inguinal hernia is usually made through history and physical examination findings. Although data are limited, in one report, the sensitivity and specificity of the physical examination were 75% and 96%, respectively.8

Symptoms of an inguinal hernia may appear gradually over time or develop suddenly, as with incarceration (i.e., the contents of the hernia sac cannot be returned to the abdominal cavity). Inguinal hernias may be asymptomatic and found incidentally on routine physical examination. Symptomatic patients often present with groin pain, which can be severe. Stretching or tearing of the tissue at and around the hernia defect can lead to a burning, gurgling, or aching sensation in the groin. This usually causes localized pain directly at the site of the hernia. Pain may worsen with Valsalva maneuvers. Patients may experience a heavy or dragging sensation in the groin, especially toward the end of the day and after prolonged activity.1 Activities that increase intra-abdominal pressure, such as coughing, lifting, or straining, cause more abdominal contents to be pushed through the hernia defect. As this occurs, the bulge of the hernia gradually increases in size. If the patient indicates that this bulge disappears while he or she is in the supine position, clinical suspicion of a hernia should be increased. [ corrected]

Hernias may be easily diagnosed with an adequate physical examination. The physical examination should begin by carefully inspecting the femoral and inguinal areas for bulges while the patient is standing. Then, the patient should be asked to strain down (i.e., Valsalva maneuver) while the physician observes for bulges. This may be accomplished by using the right hand to examine the patient's right side and the left hand to examine the patient's left side. The physician invaginates the loose skin of the scrotum with the index finger on the ipsilateral side of the patient, starting at a point low enough on the scrotum to reach as far as the internal inguinal ring. Starting on the scrotum, the examining finger follows the spermatic cord upward above the inguinal ligament to the triangular, slit-like opening of the external inguinal ring. The external inguinal ring is medial to and just below the pubic tubercle. The inguinal canal is gently followed laterally in its oblique course. While the examining finger is in the canal next to the internal inguinal ring, the patient strains down or coughs as the physician feels for any palpable herniation.9 The diagnosis of an inguinal hernia is confirmed if an “impulse” or bulge is felt.

If no bulge is detected with a Valsalva maneuver, a hernia is unlikely. However, athletic pubalgia (sports hernia) may be considered in athletes with groin pain and no bulge. A sports hernia is not a true hernia, but rather a tearing of tissue fibers. This typically occurs in patients with a history of high-intensity athletic activity. Although these patients have typical hernia symptoms, there is no evidence on physical examination. Further follow-up and reexamination are needed to diagnose a sports hernia. Pain along the symphysis pubis suggests osteitis pubis, whereas pain along the adductor tendons suggests adductor tendinopathy.

It is more challenging to diagnose a hernia in female patients. Direct palpation with an open hand over the groin area might detect the impulse of a hernia during a Valsalva maneuver. However, further workup with diagnostic testing or referral to a surgeon is often indicated. Rarely, diagnostic laparoscopy is necessary.

Incarceration may be managed in the office setting if there is no associated pain. The standard of care is to place the patient in the Trendelenburg position while holding gentle pressure on the area for up to 15 minutes. If acute onset of groin pain occurs, the hernia may have become strangulated (i.e., the blood supply to the entrapped contents is compromised). Strangulation should be suspected in the presence of tenderness, redness, nausea, and vomiting and is a surgical emergency.10
